How the discharge transport workflow works

When a patient is cleared for discharge, the case manager or social worker calls the DachiPlus facility line — not the general booking number, but a direct number established at account setup. The case manager provides the patient name, discharge location, destination address, any mobility needs (wheelchair, walker, oxygen), and a pickup window. DachiPlus dispatches a vehicle and confirms back with an ETA, typically within 15 minutes of the call.

For hospital-to-SNF or hospital-to-rehab transfers, DachiPlus coordinates directly with the receiving facility's admission team to confirm arrival time. The patient doesn't wait in a hospital bed while coordination is sorted out — we handle it.

What billing looks like for discharge transport

Discharge transport billing depends on the patient's coverage:

  • Medicaid patients — DachiPlus bills MAS directly. The patient pays nothing and the case manager doesn't need to arrange separate payment.
  • Medicare Advantage — many Part C plans include transportation as a discharge benefit. DachiPlus verifies benefit and bills directly when coverage is confirmed.
  • Private pay — patient or family is billed. DachiPlus provides a receipt for insurance reimbursement if applicable.
  • Workers' comp or no-fault auto — billed directly to the carrier using the established claim number.

For facilities that prefer a master account arrangement, DachiPlus can bill the facility for specific trip types with monthly reconciliation and line-item invoicing.
