Cardiac Rehabilitation Transportation on Long Island
DachiPlus provides recurring transportation to cardiac rehabilitation programs across Nassau and Suffolk Counties — Phase II and Phase III cardiac rehab, 2–3 times per week. Medicaid and Medicare Advantage accepted. Call (516) 754-7777.
After a cardiac event — heart attack, bypass surgery, stent placement, or heart failure hospitalization — cardiac rehabilitation is one of the most effective interventions for long-term recovery. Phase II and Phase III cardiac rehab programs typically run 2–3 sessions per week for 12–36 sessions. Missing sessions delays recovery. DachiPlus provides standing-order recurring transport to all major cardiac rehab programs across Long Island, with drivers who understand post-cardiac mobility limitations and pacing needs.

- ✓ Let us know if you use nitroglycerine or have any emergency protocols the driver should know

FAQ
Cardiac Rehab Transport — common questions
Does Medicaid cover transportation to cardiac rehabilitation?
+
Yes. New York Medicaid covers non-emergency medical transportation to cardiac rehabilitation programs for eligible members. Phase II cardiac rehab sessions typically run 2–3 times per week, so ask your cardiac rehab program to request a standing order through MAS and to list DachiPlus as your preferred transportation provider. Only your medical provider's office can enter a standing order. Until it is in place, book each session with MAS at 1-844-666-6270 at least 72 hours ahead.
How often does cardiac rehabilitation require transportation?
+
Phase II cardiac rehabilitation programs typically run 2–3 sessions per week for 12–36 sessions over several weeks. Phase III maintenance programs may continue indefinitely. For Medicaid riders, the rehab program can request a standing order through MAS so you do not need to rebook each session. A standing order lasts up to six months and does not renew on its own, so ask the program to renew it if your sessions continue.
